Biography

Valerie Smith is a Professor in Midwifery at the School of Nursing and Midwifery, Trinity College Dublin, and a registered General Nurse, Midwife and Clinical Nurse Teacher. Valerie's research interests extend to all areas of maternity care and healthcare research methods, with specific topic expertise in the areas of antenatal and intrapartum fetal wellbeing (fetal heart rate monitoring and fetal movements), intrapartum care (mode of birth, perineal care) and postnatal morbidity. Valerie's areas of methodological research expertise include systematic reviews of trials/quasi-trials and observational studies, qualitative evidence synthesis, randomised trial research, survey research, and qualitative research methods. Valerie is a Cochrane Ireland and UK Senior systematic review trainer, a member of two Health Research Board Clinical Trial Networks (Trials Methodology Research Network, Mother and Baby Clinical Trials Network and Irish Primary Care Trials Research Network), a member of the Executive Committee of Evidence Synthesis Ireland, a member of the Trial Methodology Research Partnership (Outcomes Working Group) and a member of a previous EU COST Action (ISO907) Childbirth Cultures, Concerns and Consequences, Creating a Dynamic EU Framework for Optimal Maternity Care).

All areas of maternity care with specific interest and expertise in the areas of preterm birth, antenatal and intrapartum fetal assessment (monitoring the fetal heart rate and fetal movement monitoring in pregnancy) and intrapartum care (caesarean section, vaginal birth after caesarean section and salutogenic labour and birth). Valerie's areas of research methodological expertise include survey research, randomised control trials, including methods for improving the design and conduct of trials) and systematic review methodology.

    • Improving the organisation of maternal health service delivery, and optimising childbirth, by increasing vaginal birth after caesarean section (VBAC) through enhanced women-centred care: The OptiBIRTH Project

    • The study addresses the widespread concern over rising Caesarean section rates throughout Europe, particularly routine CS following a previous CS, despite calls for increased vaginal births after Caesarean (VBAC). VBAC is associated with lower maternal mortality than repeat CS, less overall morbidity for mothers and babies, and is the preferred option for the majority of women. VBAC rates in Ireland, Germany, and Italy are significantly lower (29-36%) than those in the Netherlands, Norway, and Sweden (45-55%). The OptiBIRTH project will use a cluster randomised trial in Ireland, Germany, and Italy, with 15 clusters of 94 women in each, to increase VBAC rates from 33% to 53% through a complex intervention designed to increase women-centred care and women's involvement in their care
